Home health OASIS: what agencies collect, and what Neural Hub does not claim
Updated 2026-10-17
OASIS is a data set inside the comprehensive assessment
The Outcome and Assessment Information Set (OASIS) is a group of standard data elements that Medicare-certified home health agencies, and certain Medicaid home health providers, collect as part of the comprehensive assessment and report to CMS. OASIS is not the comprehensive assessment by itself. The assessment supports the plan of care.
CMS states that OASIS-E2 is the current version, effective April 1, 2026. Collection applies to skilled Medicare and Medicaid patients age 18 and older, with stated exclusions that include patients under 18, maternity services, and patients who receive only personal care, homemaker, or chore services. Agencies that must meet the Medicare home health Conditions of Participation are the ones the requirement attaches to. This page is not an OASIS item guide.
Why agencies ask software about OASIS
Selected OASIS items feed quality reports and payment. An agency evaluating software usually wants to know whether clinicians can complete the assessment in the record, whether the item wording and skip logic stay intact, and how the export reaches CMS. Those are reasonable questions. A vendor answer has to be specific to a named workflow.
Medicare payment for home health also depends on physician or allowed-practitioner certification under 42 CFR 424.22, including homebound status, a plan of care, and a qualifying skilled need. OASIS does not replace that certification.
What MASE says about Neural Hub
Neural Hub is MASE's clinical platform for home health and related care teams. Its product site is https://www.maseneural.io. On this website it is described at https://www.mase.health/neural-health. It is a separate product from Health OS, which is the behavioral health and OTP record.
MASE does not claim on this site that Neural Hub submits OASIS-E2, calculates a payment group, or replaces the comprehensive assessment. Asking for a demo is the way to see what the current product does: https://www.mase.health/request-demo.
